First Appointment Checklist

X-Rays

If you have had dental x-rays in the past and you have copies, bring them with you—especially if they are recent (within the past year). These records will allow Dr. Louis K. Cheung to see any developments or long-term affectations to your mouth.

We have provided a New Patient Form online for your convenience before your appointment. If you prefer to fill out the form in person, be sure to take a look at it before your appointment to have the information you will need.  Click here to download forms.

Medical History

If you take any medications regularly, be sure to bring the name. Have your, or your child’s, medical history available, including surgeries and especially any previous dental work.

Behavior

While this applies more to your child, know any behavior that could be affecting dental health. Does your child suck their thumb or use a bottle? Do you clench your teeth? Even if you think something may be insignificant, you never know what could affect your health.
